What the Job is
We seek a focused, committed RN Case Manager to join our Ethel Lund Medical Center (ELMC) in Juneau. ELMC offers the spectrum of care for children and adults, including acute and chronic disease, preventative services, maternity and pediatric care. This Case Manager Rn will collaborate and facilitate services with the medical provider and interdisciplinary team members. This position participates in case management reviews of high need patients with provider teams and advocate for patient/client family at service-delivery level and problem solve patient and the provider needs. The Case Manager RN will facilitate and track patient referrals and outcomes; Perform various nursing duties in clinics and specialties including the primary case manager role for OB patients at Ethel Lund Medical Center; Continuously develop and improve case management at ELMC.

Who You Are
The best fit candidate enjoys being an active and integral liaison among the Medical Center staff and patients. A problem solver who enjoys pivoting through a busy, often fast paced environment. A multi-tasker, you must be physically able to get through long periods of standing and moving. A natural collaborator, you partner with patients, their families and the various team members at Ethel Lund Medical Center. Two years of general nursing experience with a minimum of one year in ambulatory setting or one year case management experience. An accredited Nursing school graduate (Bachelor’s degree preferred). Must learn SEARHC services and practices as well as other Alaska tribal health organizations. A minimum of at least one (1) year of general nursing with 2 years of general nursing with a minimum of 1 year in an ambulatory setting or one year case management experience. RN licensed, with the ability to obtain Alaska License within 60 days of employment. BLS certification preferred, National Case Management certification preferred.
